1. I am an adult at least 18 years of age, and I understand valid photo ID showing birth date is required in order to gain admittance and to remain on the premises.

2. I have been provided with and have read the CSPC's Essential House Rules and agree to abide by those rules.

3. I understand that certain activities are prohibited or restricted, and are listed in the section of the above document titled Prohibited and Restricted Activities. I understand that these activities include: 

Prohibited Activities: 

• Fire place, fire cupping, and candle-wax play
• Solid state power supply electrical "wands" 
• Significant oxygen deprivation
• Intentional significant carotid artery compression
• Piss and scat play

Restricted Activities (EC Permission Required): 

• Suspension
• Blood play
• Knife play
• Takedown resistance or "forced" sex play 
• Loud, potentially disruptive, or potentially messy scenes

4. I understand that I may be asked to leave the premises for failure to abide by these rules and agreements.

5. I am aware of the nature of these events. I am not offended by the nature of this organization, I am attending of my own free will and for my own personal interest, and I understand that I am free to leave at any time.

6. I agree that all activities I engage in on the premises or at offsite events will be done with the full and informed consent of all persons involved.

7. I am not acting in the capacity of, as a member of, or under the direction of, any media, law enforcement or postal agency. I am not attending any of these events for the purpose of entrapment or to gather information and/or testimony that would lead to (or further) the arrest, prosecution or defamation of the organizers of these events, the owners of the premises, or any individual attending these events.

8. I understand that The Center for Sex Positive Culture reserves the right to modify this agreement as deemed necessary. Any substantive changes will be announced and posted in plain sight on the premises. I understand that I may not receive personal communication with regard to these changes.

9. I understand that The Center for Sex Positive Culture reserves the right to refuse service at any time.

In consideration of the services of Center for Sex Positive Culture, their agents, owners, officers, volunteers, participants, employees, and all other persons or entities acting in any capacity on their behalf (hereinafter collectively referred to as "CSPC"), I hereby agree to release, indemnify, and discharge CSPC, on behalf of myself, my spouse, my children, my parents, my heirs, assigns, personal representative, and estate as follows:

1. I acknowledge that my participation in CSPC-related activities entails known and unanticipated risks that could result in serious physical or emotional injury, paralysis, death, or damage to myself, to property, or to third parties. I understand that such risks simply cannot be eliminated without jeopardizing the essential qualities of the activities. The risks include, among other things: Strains, sprains, cuts, bruises, fractures, and other musculoskeletal injuries; injuries to internal organs; sexually transmitted disease; the negligence of other people participating; my own physical condition and the physical exertion associated with the activities. Furthermore, CSPC has a difficult job to perform. They seek safety, but they are not infallible. They might be unaware of your fitness or abilities. They might misjudge your ability to participate in activities. They might give incomplete warnings or instructions, and the equipment being used might malfunction.

2. I expressly agree and promise to accept and assume all the risks existing in this activity. My participation in this activity is purely voluntary, and I elect to participate despite the risks.

3. I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless CSPC from any and all claims, demands, or causes of action, which are in any way connected with my participation in this activity or my use of CSPC's equipment or facilities, including any such claims which allege negligent acts or omissions of CSPC.

4. Should CSPC or anyone acting on their behalf, be required to incur attorney's fees and costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.

5. I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating, or else I agree to bear the costs of such injury or damage myself. I further certify that I am willing to assume the risk of any medical or physical condition I may have.

6. If I file a lawsuit against CSPC, I agree to do so solely in the state of Washington, and I further agree that the substantive law of that state shall apply in that action without regard to the conflict of law rules of that state. I agree that if any portion of this agreement is found to be void or unenforceable, the remaining document shall remain in full force and effect.

By completing this event registration, I acknowledge that if anyone is hurt or property is damaged during my participation in this activity, I may be found by a court of law to have waived my right to maintain a lawsuit against CSPC on the basis of any claim from which I have released them herein. I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
